You’ll get up to five points extra credit for completing a course evaluation survey, which I will post the last week of class (finals week). This is mine (of course I strong urge you to complete the university’s version of the course evaluation, too).

I’ll send out a link to the evaluation on Tuesday of finals week, and students will have until Friday midnight to complete it. 

Responses are anonymous—I just see that you’ve completed the survey, not what your responses are. I don’t look at these until after grades are submitted–I do this to see how you are doing, and how you think I am doing. Points are based on the percent of the class that responds–so five points if everyone responds, four points if it’s 80%, etc.
